The liver cleanse industry is booming, but science tells a different story. Despite marketing claims about eliminating toxins and enhancing liver function, commercially available liver detox products have limited scientific evidence supporting their effectiveness—and they may actually cause serious harm to the organ they claim to protect.
The global liver health supplements market reached $10.3 billion in 2024 and is projected to hit $17.1 billion by 2034, driven by increasing rates of chronic liver disease and consumer demand for natural health solutions. However, a recent study published in The American Journal of Gastroenterology reveals a troubling gap between marketing promises and scientific reality.
What's Actually in Those Liver Cleanse Products?
Researchers analyzed the top 20 liver cleanse supplements sold on Amazon that claimed to "eliminate toxins," provide "liver detox/cleanse," or "enhance liver function." The most common ingredients included:
- Milk Thistle: Found in 19 of the 20 products examined, despite lacking high-quality evidence for liver benefits
- Dandelion and Turmeric Root: Each appeared in 13 products, though scientific support remains limited
- Other Popular Ingredients: Zinc, beetroot, artichoke extract, choline, ginger, chicory extract, and berberine rounded out the common components
"None of the top ingredients have Level I evidence—the highest quality evidence—to demonstrate their efficacy, and the majority have only limited or very limited evidence," explains Dr. Tinsay A. Woreta, associate professor of clinical medicine at Johns Hopkins School of Medicine.
Why Liver Cleanses Can Actually Harm Your Liver?
The irony is stark: products marketed to help your liver can actually damage it. Herbal and dietary supplements now account for 20% of drug-induced liver injury cases in the United States. These injuries can progress to severe acute liver injury or acute liver failure, potentially requiring liver transplant or resulting in death.
People with existing chronic liver disease face even greater risks. They're more susceptible to drug-induced liver injury and have shown greater risk for poor outcomes, including death or liver transplant. The supplement industry operates under the Dietary Supplement Health and Education Act of 1994, which allows products to enter the market without Food and Drug Administration (FDA) approval and doesn't require listing all ingredients and contaminants.
What Medical Advances Are Actually Helping Liver Health?
While the supplement industry makes questionable claims, legitimate medical breakthroughs are transforming liver disease treatment. In 2025, several significant developments emerged in hepatitis care:
The FDA granted Breakthrough Therapy designation to brelovitug for chronic hepatitis D, following phase 2 data showing 100% virologic response and up to 78% combined virologic response with normalized liver enzymes. Additionally, glecaprevir/pibrentasvir became the first direct-acting antiviral therapy approved to treat patients with acute hepatitis C in just 8 weeks, achieving a 96% cure rate.

"An ounce of prevention is worth a pound of cure should be food for thought for consumers when it comes to maintaining liver health—rather than seeking quick fixes and 'detox' strategies after the damage has already been done," advises Dr. Woreta.
