RSV Can Trigger Severe Bacterial Infections in Children, New Study Reveals
Respiratory syncytial virus (RSV) can trigger serious bacterial infections in children by making them vulnerable to invasive pneumococcal disease (IPD), according to new research published in The Journal of Infectious Diseases. The findings suggest that certain pneumococcal strains pose greater risk during RSV season, which could reshape how doctors prevent and treat these dangerous secondary infections.
What Is Invasive Pneumococcal Disease and How Does RSV Play a Role?
Invasive pneumococcal disease occurs when the bacterium Streptococcus pneumoniae moves from the upper throat into the bloodstream or other parts of the body, causing serious illness. Normally, this bacterium lives harmlessly in the nose and throat. But respiratory infections like RSV can trigger the transition from harmless colonization to invasive disease.
Researchers from France analyzed 15 years of surveillance data spanning 2008 to 2023, tracking 4,574 cases of invasive pneumococcal disease in children under 15 years old. They compared how many cases occurred during RSV season versus non-RSV season to understand which bacterial strains became more dangerous when RSV was circulating.
Which Pneumococcal Strains Pose the Greatest Risk During RSV Season?
The research revealed a striking pattern: the distribution of pneumococcal strains causing invasive disease shifted dramatically depending on whether RSV was circulating. Three serotypes, or bacterial variants, accounted for more than 30% of all cases: serotypes 24F, 19A, and 1.
Interestingly, some strains actually became more likely to cause invasive disease during the non-RSV season, suggesting that RSV's presence changes which bacterial strains pose the greatest threat. The researchers identified six serotypes with higher odds of causing invasive disease when RSV was not circulating, including serotypes 1, 17F, 7F, 38, 8, and 12F.

The study's authors emphasized the clinical importance of their findings. "Overall, this serotype-specific association with RSV may have important clinical consequences, some low disease potential serotypes being more likely to induce an invasive disease during RSV epidemics," the researchers noted. "These findings may also have important implications for RSV preventive strategies in early childhood".
The research suggests that doctors may need to consider RSV circulation patterns when assessing a child's risk for invasive pneumococcal disease. During RSV season, certain bacterial strains become unexpectedly dangerous, even though they typically cause less severe disease. This knowledge could help clinicians make more informed decisions about monitoring and treatment.
As RSV vaccines become more widely available and RSV prevention strategies evolve, understanding how RSV interacts with pneumococcal disease becomes increasingly important.
