GLP-1 Drug Tirzepatide May Cut Heart Attack Risk by a Third in People With Diabetes

A major new study suggests that tirzepatide, the active ingredient in the popular weight-loss and diabetes drugs Mounjaro and Zepbound, may significantly lower the risk of heart attack in people with type 2 diabetes and established heart disease. Researchers analyzing health insurance data from nearly 53,000 people found that those taking tirzepatide had a 33% lower risk of heart attack compared to those taking a different diabetes medication, with one major cardiovascular event prevented for every 70 people treated.

What Did the Research Show About Heart Protection?

The study, published in The BMJ, compared tirzepatide with sitagliptin, another diabetes medication known to have no effect on cardiovascular outcomes. Over one year, the tirzepatide group experienced a 2.9% risk of major cardiovascular events (which include heart attack, stroke, and death from any cause), compared to 4.4% in the sitagliptin group. The research included 52,971 individuals with an average age of 70, of whom 35,353 started tirzepatide and 17,618 started sitagliptin.

Beyond heart attack prevention, the tirzepatide group also saw reductions in other serious health outcomes. The medication was linked to one hospitalization for infection prevented for every 48 people treated, one death from infection prevented for every 200 people, and one death from any cause prevented for every 122 people.

How Might Tirzepatide Protect the Heart?

Tirzepatide works by mimicking two natural gut hormones: GLP-1 (glucagon-like peptide-1) and GIP (glucose-dependent insulinotropic polypeptide). This dual action helps lower blood sugar and supports the body in producing more insulin when needed, while also slowing digestion to help maintain stable blood sugar levels after eating.

The cardiovascular benefits likely stem from multiple mechanisms working together. According to experts, GLP-1 medications like tirzepatide can improve heart health in several key ways:

  • Blood Sugar Control: The medication helps manage blood glucose levels, a major risk factor for heart disease in people with diabetes.
  • Weight Reduction: Tirzepatide promotes significant weight loss, which reduces strain on the cardiovascular system and improves overall metabolic health.
  • Blood Pressure and Cholesterol: The drug may favorably influence blood pressure and lipid levels, both critical factors in preventing heart attacks and strokes.
  • Anti-Inflammatory Effects: GLP-1 medications can reduce inflammation throughout the body and improve blood vessel function, supporting better circulation.

Notably, the cardiovascular benefit curves began to separate relatively early in the study, before substantial weight loss would typically occur. This suggests that direct vascular and anti-inflammatory effects may play a role alongside weight loss benefits.

"Tirzepatide improves glucose control, reduces body weight, and may favorably influence blood pressure, lipids, inflammation, and other cardiometabolic risk factors. What Are the Study's Limitations and Next Steps?

While the findings are encouraging, researchers acknowledge important limitations. The study was observational rather than a randomized controlled trial, meaning it tracked real-world outcomes but cannot definitively prove that tirzepatide caused the reduced heart attack risk. Additionally, the follow-up period lasted only one year, so longer-term benefits remain unclear.

It is important to note that tirzepatide does not currently have FDA approval specifically for reducing cardiovascular risk. Mounjaro is approved only for type 2 diabetes treatment, while Zepbound, which contains the same active ingredient, is approved for weight loss. Coverage and usage remain limited to patients with type 2 diabetes, obesity, and obstructive sleep apnea.

Researchers plan to investigate whether these cardiovascular benefits persist over longer follow-up periods and can be replicated in other populations and healthcare systems. They also want to better understand the mechanisms behind the reductions in infection-related events and determine how tirzepatide compares directly with other cardiometabolic therapies.
