Baker's Asthma Is More Common Than You'd Think,Here's What Workers Need to Know

Baker's asthma, also called baker's lung, is an occupational respiratory disease affecting 7% to 15% of bakers who are repeatedly exposed to airborne flour dust and baking ingredients. Though the condition has been documented since the 1700s, it wasn't until the 1970s and 1980s that scientists could measure the specific irritants and allergens causing it. Today, it remains one of the most common but least discussed occupational respiratory diseases, even as preventive measures have improved.

What Causes Baker's Asthma to Develop?

Baker's asthma develops after repeated inhalation of flour dust and other airborne substances commonly found in bakeries. Unlike simple irritation, baker's asthma involves both an irritant and allergic reaction in which the immune system becomes sensitive to specific proteins. Once sensitization occurs, even small amounts of exposure can trigger symptoms.

The culprits are surprisingly varied. Workers may be exposed to flour and grain dust from wheat, rye, barley, soy, or buckwheat, as well as fungal alpha-amylase, an enzyme frequently added to flour to improve baking performance. Other additives and enzymes including eggs, egg powder, sesame seeds, yeast, and nuts can also trigger the condition. Storage grain mites and molds present additional hazards. Since workers are often exposed to multiple ingredients simultaneously, it can be difficult to pinpoint a single trigger.

Certain occupations carry significantly higher risk. Workers involved in mixing, weighing, pouring, or sifting flour have the highest exposure because these tasks generate large amounts of airborne particles. Other factors, like a history of allergic conditions or tobacco use, have also been associated with increased sensitivity.

How Does Baker's Asthma Start and Progress?

In most cases, before workers develop baker's asthma, they often experience occupational rhinitis or eczema, or dermatitis. This may result in cold-like symptoms such as a runny nose, sneezing, and congestion. If symptoms are addressed promptly, developing baker's asthma may be prevented or controlled. If not, people may experience wheezing, persistent cough, chest tightness, shortness of breath, and itchy, watery eyes.

"Occupational history and exposure history is important when diagnosing a patient who comes in with wheezing, cough, sputum production and chest tightness. It's also instructive to note if the symptoms are better on weekends, for example, if the patient is removed from the exposing agents," explained Dr. Glenn Singer, a pulmonologist.

Dr. Singer also noted that prolonged exposure is typically needed for baker's asthma to develop. "Usually, prolonged exposure is needed for baker's asthma. Casual home exposure could potentially cause a problem, but that would be less likely," he stated.

Which Workers Are at Highest Risk?

While anyone who regularly works around airborne flour dust can develop baker's asthma, certain occupations face significantly higher risk. At-risk professions include:

  • Commercial and artisan bakers: Workers in traditional and industrial bakeries face constant exposure to flour dust and baking ingredients.
  • Pastry chefs: Professionals working with flour-based doughs and batters encounter similar hazards as bakers.
  • Bakery assistants: Support staff handling ingredients and cleaning equipment are regularly exposed to airborne particles.
  • Flour mill workers: Employees in grain processing facilities face high concentrations of flour dust.
  • Pizza makers: Workers tossing and handling dough generate significant airborne flour particles.
  • Food manufacturing employees: Those working with flour in pet food production and other food manufacturing face occupational exposure.
  • Supermarket bakery staff: Employees in retail bakery departments handle flour and baking ingredients daily.

How to Prevent and Manage Baker's Asthma

Baker's lung is highly preventable with proactive steps. Once diagnosed, the condition persists, so prevention and early intervention are critical. "Unfortunately, once the condition is diagnosed, it's there. While there are treatments, avoidance is the best strategy," Dr. Singer explained.

Eliminating or reducing flour dust is the best way to reduce harm. For individuals working in at-risk industries, preventive measures include:

  • Reduce airborne particles: Switching to low-dust products, enclosing storage containers, pouring ingredients gently, mixing slowly, and avoiding shaking flour bags can all reduce particles in the air.
  • Improve ventilation: Installing local exhaust fans to vent flour particles to the outside helps remove contaminants from the breathing zone.
  • Use proper cleaning methods: Vacuum systems with High Efficiency Particulate Air (HEPA) filters or damp cloths capture particles safely without sending them back into the air.
  • Maintain equipment: Performing regular environmental health and safety checks ensures machinery is clean and in working order.
  • Wear protective equipment: Using personal protective equipment like masks helps avoid inhaling dust during high-exposure tasks.

Despite these preventive measures, exposure cannot be completely eliminated. If you have asthma, monitoring and reporting any increase in symptoms is essential. One effective strategy is keeping a symptom diary to identify patterns and determine when symptoms worsen or improve. Always have your Asthma Action Plan and quick-relief rescue medicine on hand in case of an emergency, and follow your plan closely. Alert your employer and healthcare provider if you are experiencing a flare-up.
